I hereby authorise the Team Coach or Club Official in the event of being unable to contact emergency contact to obtain medical assistance as deemed necessary. Sorrento Netball Club agrees not to use photos in a manner that may be deemed adverse or defamatory manner. Photos or video will remain the property of Sorrento Netball Club and personal details regarding the material will be kept confidential.
